On my INCOMPLETE REPORTING/AMENDMENT FORM it is asking for the work sample we forgot to send in for math, but it also has the parent signature space highlighted yellow [indicating I need to sign it]. Did I not sign the Semester Report form when I sent it in?


Yes, you did sign your original Semester Report Form. The signature request on your Amendment Form is standard procedure and separate from your original signature.


Amendment Forms always require a parent signature to authorize adding new information to your student's original Semester Report. This happens even when your original form was properly signed.


If you hadn't signed the original Semester Report Form, we would have sent you a different message stating that we cannot begin reviewing your student's packet until we receive a complete, signed Semester Report Form. Since you received an Amendment Form instead, you can be confident that your original signature was received.


Simply sign the Amendment Form and return it with the missing math work sample to complete your student's reporting packet.
