If you have received an Amendment Form, it means one or more pieces in your student’s semester reporting packet were missing or unclear. This article explains what the Amendment Form is, how to complete it, and where to find helpful resources in the STAA Handbook.
What Is an Amendment Form?
Amendment Forms have bold, yellow highlights and comments requesting the additional information or work samples necessary to complete your student’s reporting packet. Until the Amendment Form is completed and returned, your student’s transcript is marked incomplete, and an official copy is not available.
Please look over the Amendment Form so you can discuss it with your advisor during your upcoming Transcript Review meeting. Your Transcript Review appointment was scheduled when we received your semester reporting information.
How to Complete the Amendment Form
Please complete all yellow portions of the form with a bold, dark pen. Follow these guidelines:
- Do not leave any yellow portion of the form blank.
- Write DROP on any course that you have decided not to report for grades and credits.
- Write DEFER on any course that will be studied in a future semester. This will drop the course from the current semester and defer it to the next semester. All of the course’s work samples will need to be submitted at that time.
You are very welcome to wait to discuss the Amendment Form with your advisor before you submit it. Your advisor will offer suggestions and explanations for how to complete the fields on the form that are highlighted yellow, as well as offer advice about what to submit if the form indicates a work sample is needed.
How to Submit Your Amendment Form and Requested Documents
The Amendment Form and any gradebooks, standardized test results, or transcripts from colleges should be submitted through the “Special Enclosures” space on the Semester Reporting course page. Sign in as the student to upload these documents.
Work samples for STAA Study Guide courses should be submitted through their course pages. Work samples for courses without STAA Study Guides should be submitted in the subject spaces on the Semester Reporting course page (the same page where you uploaded your Semester Report Forms and Amendment Form).
Please use a scanner or scanning app to convert documents into PDF files (no larger than 5 MB each) before uploading them to the STAA Student Zone. Please avoid sending documents by email.
How to Access Your Amendment Form in Your Private Folder
To view and download the Amendment Form, sign into the STAA Student Zone at www.staahomeschool.com/studentzone, click on the Profile in the top right corner, and then click the “PRIVATE FOLDER” link on the profile page.
- When you sign in as a student, the PRIVATE FOLDER link will open only the student’s folder.
- When you sign in with your teaching-parent account, the PRIVATE FOLDER link will open your family’s folder, where you will find your student’s folder as a subfolder.
Your private student folders are maintained by St. Thomas Aquinas Academy, so you do not need to create a Box.com account to access them. If you have misplaced your STAA Student Zone username and password, you can find help here: Web Logins.

Sending Official Transcripts
If you would like us to send an official transcript to a college or other institution on your student’s behalf, please submit a transcript request by visiting staahomeschool.com/transcript-request. This link is also available on the Contact Page of our main site and in the main menu of the STAA Student Zone.
Be sure to review the latest version of the transcript in your student’s private folder before placing the transcript request. If your student’s transcript is presently marked incomplete, we will place a hold on the transcript request and send an official copy of the transcript as soon as you submit the Amendment Form (and any requested work samples).
